2021 Annual Meeting Proxy Form

If you are not able to make it to the 2021 Annual Meeting, please submit a proxy form. Assigning a proxy allows another resident to vote on your behalf. A co-owner cannot represent more than one proxy, so please speak with your neighbor prior to assignment.

Use this form to submit a Work Order to the office.  Remember to include your unit number with your request.  Maintenance staff will contact you if there are any additional questions.  For emergencies, please contact a Board Member.

If you are a current resident, please refer to the Owner's Packet and Rules and Regulations to answer most questions. 
  • Can units be rented?
    • Units cannot be rented nor leased for any amount of time.  Airbnb and similar space-sharing arrangements are not allowed.  All units must be owner-occupied.
  • When are condo fees due?
    • Condo fees are due on the 15th of the month for the current month.  You can pay condo fees in advance.  If you pay by ACH, we will deduct your condo fee on the 14th.
  • How can I pay my condo fee?
    • Automatic bank account draft, check, or money order.  We do not accept cash or cards at this time.  Drop off payments (or a voided check for ACH) in the office mailbox in the E13-E16 hallway or in the Office, located in the E basement.
  • How is Salem Square
        

    managed?
    • Salem Square does not use a management company.  The Board of Directors handles all Salem Square affairs.  The Board is staffed by five residents who volunteered to serve and were elected by residents.  Salem Square also employs a Maintenance Manager and a variety of maintenace support staff.
  • I have an emergency.  What do I do?
    • If you have an emergency that you feel requires medical, fire, or police assistance, please contact 911.  If you have a maintenance emergency that impacts your safety, home, or quality of living, please contact the office first.  If you do not get a response, leave a voicemail and contact a Board member for help.  See the Contact Us tab for contact information.
  • How do I get a condo certification?
    • First, you'll need to complete a Sales Form.  Check out the Relator Information tab for this form.  E-mai the Sales Form along with your request to SalemSquareBoard@gmail.com.  If you need a non-standard or proprietary Condo Cert, please e-mail that as well.  We are a volunter Board but try to acknowledge and complete all requests within 24 hours.
